Are you passionate about improving patient journeys and optimizing treatment plans? We are seeking a skilled and dedicated **Clinical Case Manager** to join our team in the APAC region.

As a Clinical Case Manager, you will play a vital role in managing complex clinical cases, with the dual objective of enhancing patient outcomes and optimizing treatment costs. Your expertise and strategic approach will be crucial in achieving these objectives while ensuring effective coordination across multiple teams.

**Responsibilities**:

- Take ownership of the case management process, ensuring a comprehensive understanding of each complex ongoing medical case.
- Develop innovative and effective action plans to achieve the objectives of improving patient journeys and enhancing cost-effectiveness.
- Monitor the cost savings and continuously propose new strategies to improve the savings based on the analysis.
- Focus on customer experience and provide clear and transparent explanations to members. Engage with members regarding their healthcare situations, providing guidance and discussing alternative treatments, suggesting alternatives, and facilitating second medical opinions when necessary.
- Prepare well-structured and concise case summaries as part of the escalation process, ensuring all relevant information is included.
- Oversee and coordinate the various teams involved in the case management process, including the LOG team and medical team. Align with objectives and key performance indicators.
- Organize regular case study sessions to enhance teamwork, communication, and collaboration among team members. Foster a supportive and collaborative work environment.
- Participate in a roster system to ensure seamless service continuity and availability.

**Requirements**:

- A degree in a relevant healthcare field, such as Medicine (preferred), or Nursing, Healthcare Management.
- Strong analytical and problem-solving skills, with the ability to develop innovative action plans and drive cost savings.
- Excellent communication and interpersonal skills, with the ability to engage effectively with members and medical professionals.
- Knowledge of healthcare systems and clinical procedures, with the ability to understand and interpret medical information accurately.
- Ability to work independently, prioritize tasks, and meet deadlines in a fast-paced environment.
